1. Open form by going to Reporting > Create Groups...
2. Make sure the Select Parameters tab is the active tab. The tabs are located across the top of the form.
3. A list of all available parameters will be shown by default in the middle Available list.
4. Double click on any items from the Available list that you want in your group. These items will appear in the list on the far right.
5. When finished adding items, press the Save button to save the list. You will be prompted to enter a name.
